Youth Wellness & Recovery Navigation Program (MAT)

The Youth Wellness & Recovery Navigation Program provides affirming, trauma‑informed, and harm‑reduction‑based support for youth navigating substance use, mental health, and life challenges. This program meets young people where they are rather than expecting them to navigate complex systems alone. It offers inclusive, culturally responsive care for youth of all identities, including 2S/LGBTQIA+ young people.

Community outreach and education begin through healing circles, workshops, wellness events, and stigma‑reduction campaigns that build trust and awareness of harm reduction, recovery, mental health, and Medication‑Assisted Treatment (MAT). Peer ambassadors and wellness leaders with lived experience provide relatable support through outreach, groups, and one‑on‑one navigation. Navigators and case managers connect youth directly to affirming MAT and substance‑use providers, removing barriers through warm handoffs, transportation, and follow‑up care. Ongoing case management links youth to housing, education, social services, and workforce‑development opportunities—creating a seamless pathway from engagement to recovery and long‑term stability.

What We Offer

  • Community Outreach and Education: Bi-weekly workshops, healing circles, community event tabling, Pride activities, wellness fairs, and stigma-reduction campaigns focused on harm reduction, substance use, mental health, and MAT.

  • Peer Leadership and Navigation: Peer-led outreach, support groups, one-on-one navigation, school and shelter engagement, and training for peer ambassadors and wellness leaders with lived experience.

  • Treatment Access and MAT Navigation: Plain-language education about treatment options, warm handoffs to LGBTQIA-affirming MAT and substance-use providers, partner-agency referrals, transportation assistance, and follow-up support.

  • Case Management and Holistic Support: Ongoing case management and referrals to housing, social services, education, workforce development, and other resources that promote long-term stability and wellness.

Youth ages  16–24 years old, including LGBTQIA+ youth.
