Position Summary: The Case Manager will provide intensive case management and treatment navigation services for youth impacted by substance use and incarceration. This role focuses on connecting young people—particularly those exiting juvenile detention—to Medication for Addiction Treatment (MAT), recovery supports, and holistic wellness resources. The Case Manager will use a trauma-informed, culturally affirming approach to reduce stigma, address barriers to care, and promote long-term stability and recovery.

Key Responsibilities

  • Conduct intake assessments and develop individualized service and recovery plans.

  • Provide warm hand-offs to MAT providers, behavioral health services, housing, and recovery resources.

  • Support youth in scheduling, attending, and following up on treatment and medical appointments.

  • Conduct outreach in the community and at juvenile detention centers to engage eligible youth.

  • Facilitate workshops and stigma-reduction education related to substance use and wellness.

  • Collaborate with probation, schools, providers, and community partners for wraparound support.

  • Maintain accurate case notes and data entry in line with program and funder requirements.

Qualifications

  • Bachelor’s degree in Social Work, Public Health, or related field OR equivalent lived/professional experience.

  • 1–2 years’ experience working with youth impacted by incarceration, homelessness, foster care, or substance use.

  • Knowledge of Medication for Addiction Treatment (MAT), harm reduction, and recovery models.

  • Strong commitment to culturally affirming, trauma-informed, and inclusive practices.

  • Excellent communication, organizational, and advocacy skills; ability to work evenings/weekends as needed.

  • Preferred: Bilingual (Spanish/English), lived experience, or certification in recovery/harm reduction.
